Prior to Your Appointment
After scheduling your appointment, you will receive an email with directions to register for the patient portal. In the patient portal, you will be able to fill out all of your necessary paperwork for your upcoming appointment, including your
health history. We ask all patients to try to complete this prior to their appointment, as it will save you a lot of time the day of your appointment.
If you have had any previous imaging done, you may have the report faxed to our office at (720) -370-9545. Please bring the images and report with you on the day of your appointment. This will help to speed up the process of gathering information
in order to secure a correct diagnosis.
