The Sports Medicine System of Care
The FORM Medical team is led by Jessica Kelner, DO. Dr. Kelner is a dual board-certified physician in both Neuromusculoskeletal Medicine as well as Family Medicine and OMT. Dr. Kelner’s goal is to help people become and stay healthy. At FORM Medical, we utilize a comprehensive system of care, working closely with Physical Therapists and Athletic trainers to achieve maximum results. Should surgical intervention be required, Dr. Kelner will coordinate care with appropriate orthopedic specialists.
